As a reminder, there is a special enrollment period (SEP) for beneficiaries who have a severe or disabling condition and live in a service area where a chronic condition special needs plan (C-SNP) serves people with the condition.
- For Peoples Health, beneficiaries who have diabetes, heart failure or a cardiovascular disorder can use this SEP one time per qualifying condition to enroll in a Peoples Health C-SNP at any point during the calendar year.
- For example, a beneficiary diagnosed with diabetes can use the chronic condition SEP to enroll in a Peoples Health C-SNP.
- If the beneficiary leaves the C-SNP but later wants to re-enroll, they’re not eligible to use the diabetes diagnosis to enroll.
- However, if they have another qualifying condition, such as heart failure, they do qualify to enroll based on the heart failure diagnosis.
- A health care provider must verify that the beneficiary has a qualifying condition for the beneficiary to remain enrolled in the plan. If a provider doesn’t verify the condition, the beneficiary will be disenrolled from the C-SNP and covered by Original Medicare.
